view your health information online. All you need is Internet access and e-mail.

 With this helpful online tool, you can view portions of your medical record and perform other functions — when it's convenient for you. With MyChart, you can:

• Review your current health issues, medications, immunization and allergies
• View test results
• Renew medication prescriptions
• View your medical history
• View your family’s medical history (special proxy access required)
• Communicate with your providers office
• Request, cancel and view appointments

It's easy to enroll. Download the enrollment form and release that require your signature or pickup at any West Bend Clinic location. Once completed, you will receive an access code that will allow you to setup your MyChart account.
